Your immigration physical covers all USCIS-required I-693 components: physical examination, TB blood testing, syphilis and gonorrhea screening (ages 15+), mental and physical health review, drug use assessment, and a vaccination evaluation covering MMR, Tdap, hepatitis A, hepatitis B, influenza, and others. All vaccines are kept in-stock for your convenience.
Passport Health recommends doing the following for your appointment:
- Vaccination records (from any country)
- Prior TB treatment records or chest X-rays, if applicable
- Government-issued photo ID
- Completed Part 1 of Form I-693
Your completed I-693 is provided in a sealed envelope for Form I-485 submission and an unsealed copy for your personal records. All fees are due at time of service; cash and major credit cards accepted.

Pricing can vary based on a variety of factors including which tests are needed and if you need to receive any vaccinations. Passport Health does provide an itemized invoice in case you’d like to submit any vaccinations to insurance for reimbursement.
